Orland Park Dental Services will provide dental services for eligible students. These services may include an exam, cleaning, fluoride treatment and sealants (a protective coating on the chewing surfaces of back teeth). Licensed dentists, hygienists and assistants will come to our school Monday, December 20 and Tuesday, December 21 with portable equipment.
In order for your child to receive these services, you must provide all the information requested here for online registration. You must complete a form per student. Also, it is important to note that all of our students qualify for free meals, so please mark yes on the form. Please notify the main office if you prefer a hard copy of the registration form.
